For new clients who need to fill out paperwork:
We ask that you arrive 15 minutes prior to your appointment. This gives you time to fill out the forms (please click the FAQ's button below for information about forms and paperwork), ask any questions you may have and also allows time for your Therapist to review the information and discuss any specific concerns with you. We want to get this completed before the start time of your appointment because at Summit, our treatment times are the full time indicated in their descriptions. However, if you arrive right at your appointment time and need to spend part of that time filling out forms, we cannot guarantee that you will get your entire appointment time hands on.
For established clients:
It is still a good idea to be in our waiting area before your appointment start time. Arriving 5 minutes early ensures that you could use the restroom if the need arose and you will have time to get comfortable for your treatment.
Essentially, if you are late, we cannot guarantee you will receive your full treatment time and you will be responsible for the full cost of the service.
We require 24 hours' notice for cancellations. Cancellations within 24 hours of an appointment will be charged a $25 re-booking fee payable before booking new appointment.
Before allowed to re-book, clients who "No call/no show" will be charged 50% of the original appointment price and be required to pre-pay for any future bookings.
Inappropriate behavior can include but is not limited to the insinuation, joke, gesture, conversation or request to perform anything of a sexual nature. It will not be tolerated. Your session will be terminated with no refund and you will be refused any future bookings.
For your convenience, please download any and all forms that apply to your upcoming appointment, fill them out at your leisure and bring them with you when you come in.
All new clients must fill out and sign the Massage Intake Form and the General Liability Release Form. Other forms are required for particular services; for example, Pre-Natal clients must complete the Pre-Natal Release Form in addition to the required forms. For clients receiving Kinesiotaping with RockTape™, please also download the form on how to safely remove RockTape™ if you wish to remove it before it comes off naturally.
